Registration Paperwork and Documentation Information

INSTRUCTIONS FOR HANDING IN DOCUMENTATION

  • If you were unable to make the paperwork and equipment dates, please email your forms (exept your child's birth certificate) to wwyf01778@gmail.com. 
  • We will hold another equipment hand out date in August (date TBD), which we will collect copies of your child's birth certificate. 
  • For liability purposes, there are NO EXCEPTIONS to the rules about 100% of the documentation must be turned in before your child can participate.
  • We will have Board Members and Volunteers at the table and ask that you understand these rules and prepare ahead of time.

 

EQUIPMENT INFORMATION

  • There will be a documentation table setup to hand in ALL signed and completed forms before equipment is issued.
  • Once you turn in all your forms, your child will go to the WWYF equipment box and be fitted for a helmet, shoulder pads, practice pants and jerseys.
  • If your child does not have a helmet issued by WWYF, they cannot practice.
  • First 5 practices are helmets only, regardless of when your child begins practices.

 

RETURNING PLAYERS

The good news is that much of the information you submitted last year has been saved to the paperwork you must print out and complete, located in your wwyouthfootball.com account.

If you need to make any necessary updates/changes to medical, insurance, and contact information, please log in and make changes to your child’s account first. Doing so will save you time and effort as your updates will carry over to the paperwork you need to print, review and bring with you on.

How to make changes to medical, insurance and contact information:

  1. Login to wwyouthfootball.com (Click green Log in button in upper right)
  2. Click on the Account Settings link located on the right under the Account Options
  3. Click on the name of your child under the Children section (You will need to follow these steps for each child registered)
  4. Click the Edit link located on the right under the Child Options
  5. Update information, click the Save button, and then the Done button
  6. Click the “Back to account overview” link to return to the main page
  7. Click the gray “Print Forms” box to the right of your child’s name ONLY after you’ve made any necessary updates
  8. Click to print each document
  9. Double check that all updates have been saved and reflected on the forms

NOTE: PLEASE DO NOT UPLOAD OR CHANGE YOUR CHILDS PHOTO OR IMAGE - we will be taking pictures with 2017 uniform jersey and adding to this form later)

How to print registration forms:

  1. Login to wwyouthfootball.com
  2. Click the gray “Print Forms” box to the right of your child’s name
  3. Click to print each document ONLY after you’ve made any necessary updates
  4. Double check that all updates have been saved and reflected on the forms

 

Please bring the following forms:

  1. Participation and ID Card
  2. Emergency Medical Treatment Form
  3. Copy of 2017 Physical or signed Medical Clearance Form by your doctor                               Please note: If your child’s wellness visit was in 2016, you will need the Medical Clearance form signed by your doctor.
  4. Copy of 2017 report card or progress report from spring semester
  5. Waiver and Release Form
  6. Concussion Acknowledgement Form
  7. Image Release Form
  8. Code of Conduct Form 

NOTE: We will have a copy of your child’s birth certificate from last year. Over the next few weeks, we will contact you directly if we need you to bring another copy, but most likely we have it on file.

 

NEW PLAYERS, INCLUDING FLAG FOOTBALL

Online registration may take 10-15 and you will need to print 6 or 7 documents. In addition to the printed documents, you must bring the following:

  1. An original birth certificate with the raised seal or stamp AND an extra copy of the birth certificate. (Original will be reviewed and given back to you, we will keep copy for our records).
  2. Copy of 2017 report card or progress report from spring semester
  3. Doctor’s wellness report signed and dated in 2017. Please note: If your child’s wellness visit was in 2016, you will need to bring a copy of the Medical Clearance form signed by your doctor.

HELPFUL TIPS

  • Before printing documents, please double check that all updates have been saved and reflected on the forms
  • Every parent/guardian and child participant should read the Code of Conduct and one parent and child must initial and sign that page
  • Please sign and initial in all the areas request on the forms, sometimes it is at the bottom, other forms have you initial throughout the form
  • If you have a 2016 Doctors Wellness Report, you must have your doctor sign the Medical Clearance form and bring both to the Document Table. (NOTE: some doctor’s office’s will stamp upon request and others require a one week turn around for doctor’s signature - Get started on these now!!!!)
  • Some older versions of web browsers may have trouble accessing or printing the documents so please complete your paperwork earlier enough to make alternative printing plans

We are very excited about the 2017 season.
